If you are traveling north on I-81 you would take exit #23, turn right on to 11E and get into the left lane.  Just over the hill you should turn left at the crossroad between the BP station and a semi tire store onto Gap Creek Road.  It's about 4 miles to Pilot Mt. Rd. There are 3 ways to get onto Pilot Mountain Road from Gap Creek Rd. Sometimes MapQuest or Google will list Pilot Mountain Road as Doc Self Road (It's a 911 thing. :)) You can see on the map I've attached that it shows Doc Self Rd. super imposed over Pilot Mt. Rd. We always come in via Thompkins Rd., since it is faster.

If traveling south on I-81 you would take exit #30 and turn right onto Hwy 70.  This is a two lane road.  Follow Hwy 70 for several miles.  You will encounter one very deep curve to the left.  The next land mark is a bridge over Gap Creek.  Shortly after crossing the bridge you will see a road that turns to the left.  That is Gap Creek Road.  Stay on Gap Creek for a few miles and you will see Pilot Mt. Rd. on the right.  If you miss that turn the next access road is Thompkins Rd and if you miss that you will come to another Pilot Mt. Rd.

Pilot Mt. Rd. is a one lane road.  Drive carefully.  We don't have a lot of traffic, but you never know.
